Description

Responsible for the coordination of all aspects of patient care including coordination of referrals and patient care-related supervisory functions, and collaboration with the Hospice Interdisciplinary team.

• Coordinates and manages all services rendered to patients and families by direct care staff under his/her supervision; 
• Effectively manages all day to day activities in assigned area including staff orientation, training, scheduling, and performance review;
• Provides oversight of daily schedule to ensure all patient needs are met;
• Supervises assigned nursing personnel in the delivery of nursing services to patients and families
• Directs evaluation of the clinical competence of assigned nursing personnel and participation with appropriate supervisory staff in the evaluation of other direct service staff; 
• Participates in development of all agency objectives, standards of care, policies and procedures affecting clinical services through analysis of quality outcomes and implementation of current best practice
• Participates in direct services staff recruitment, selection, orientation and inservice education;
• Participates in program planning, budgeting and evaluation activities related to the clinical services of the agency. 
• Participates in all indicated Hospital and Department meetings and inservices.
• Special projects and other job related duties as assigned.

Requirements

Minimum Qualifications:

• A graduate of an accredited Bachelor of Nursing program
• Registered nurse with an active license to practice nursing in CT. 
• Will meet the criteria for supervisor of clinical services as described in State of CT Public Health Code for Home Health Care Agency
• Three (3) years of clinical nursing experience- at least one (1) year in home health.

Preferred Qualifications:  

• Master’s of Science of Nursing 
• 1 year Hospice experience
